Job Description
Responsibilities Build and maintain positive relationships with patients, caregivers, and healthcare team members. Support patient care coordination and follow-up activities. Use IT systems to track, document, and monitor patient care and clinical indicators. Conduct basic health screenings and measurements. Promote preventive health screenings and vaccinations in line with national health initiatives. Provide basic health education on healthy lifestyles, disease prevention, and self-monitoring of health conditions. Guide patients on the use of home health monitoring devices such as blood pressure monitors, glucose meters, and weighing scales. Assist patients in accessing healthcare and community resources to improve their overall well-being. Leverage technology to enhance patient engagement, health literacy, and access to care. Other ad hoc duties assigned by the supervisor Requirements NITEC, Diploma, O-Level, or A-Level qualification. Good verbal and written communication skills. Pleasant personality with strong interpersonal skills. Positive attitude, self-motivated, and a good team player. Interested in patient care and community health. Proficient in Microsoft Office applications (Word, Excel, and PowerPoint). Comfortable using IT systems and digital tools.
