Job Description
To provide the best possible preventive, developmental, remedial and rehabilitative interventions unique to every child residing at Sunbeam Place (SBP). To deliver quality trauma-informed case management and counselling services. General Duties Work in partnership with the SBP team.
Work collaboratively with professionals from respective social work organizations, the MSF, educational institutions, hospitals and any other organizations involved in the care plan of every child.
To participate in the Children In Care Standing Committee (CICSC), Admission, Progress and Discharge Workgroup (APDW) and Care Plan Meetings, as required.
Any other admin tasks assigned by Reporting Officer Case Management Provide counselling support to assigned residents by regularly meeting them at least once a month and ensure that these sessions are maintained in written records in a timely manner.
This may involve being assigned clients presenting with more complex and greater needs, which requires more rigorous collaboration with internal and external stakeholders to implement intervention plans.
Assess residents’ overall wellbeing and formulate/implement a care plan to monitor the progress and development of the resident.
a) Recommend appropriate therapeutic intervention as part of care plan b) Monitor and record changes in resident’s behavioural performance in care plan and provide updates during care plan meetings or when necessary.
c) To develop and enhance systems to improve existing services d) Work with the Residential Care staff to ensure the implementation of care plan interventions. e) To regularly evaluate the outcome of care plans and any other intervention carried out for the residents.
