Our digital tools help healthcare providers monitor, educate, and support their patients in real time, enhancing overall care coordination and improving health outcomes. About the Role We are seeking a full-time Associate Care Manager to support patient engagement, care coordination, and remote patient monitoring efforts. This role is ideal for candidates with a background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role with experience in care coordination, patient navigation, or remote patient monitoring (RPM). The Associate Care Manager works closely with patients enrolled in Babyscripts ' RPM programs, ensuring compliance with care plans, addressing questions, and escalating concerns to clinical staff when needed. While prior experience in maternal health or OB care is a plus, it is not required. The ideal candidate is passionate about patient engagement, digital health, and improving healthcare outcomes. This is a remote position with structured yet flexible scheduling, offering an opportunity to make a meaningful impact on maternal healthcare while working in an innovative and fast-growing digital health company. Key Responsibilities Engage with patients through in-app messaging, phone calls, and telehealth check-ins to support adherence to their care plans. Monitor patient data (such as blood pressure readings) and conduct follow-ups with patients who have not submitted readings in several days. Educate patients on how to use digital health tools and properly measure vital signs. Escalate concerns related to abnormal patient readings or reported symptoms to RN Care Managers or clinical teams based on established protocols.
