Summary Coordinates pharmacy initiatives involving registry or high-risk patients identified within the LVPHO (LVPG, MATLAV and Affiliate) practices. Identifies, organizes, and completes performance improvement projects related to medication use. Develops and implements pharmacy initiatives directed at promoting high value, high quality, safe, and cost-efficient medication therapy management in accordance with value-based initiatives.
