Summary Coordinates pharmacy initiatives involving registry or high-risk patients identified within the LVPHO (LVPG, MATLAV and Affiliate) practices. Identifies, organizes, and completes performance improvement projects related to medication use. Develops and implements pharmacy initiatives directed at promoting high value, high quality, safe, and cost-efficient medication therapy management in accordance with value-based initiatives. Job Duties Applies subject matter expertise in clinical pharmacy, therapeutic alternatives, utilization management, medication adherence strategies, and Medication Therapy Management to promote positive outcomes in LVHN value-based reimbursement contract programs.
