RESPONSIBILITIES Role & Responsibilities: Reporting to the Centre Lead, the Programme Coordinator is to connect with seniors, the community and partners to empower seniors to age well in their community. Plan and conduct outreach to raise awareness about centre programmes and services and to enrol new seniors as centre members Conduct basic assessment to identify socially isolated seniors and those with needs. Collaborate with grassroots, community partners and service providers to build up community network, which includes initiating and coordinating community engagement efforts Active Ageing to encourage active living and empower seniors. Organise activities at Eldercare centre and off-site locations. Co-create activities with volunteers. Empower seniors to be volunteers. Befriending and Buddying to prevent social isolation. Manage volunteers to provide befriending/buddying services. Befriend seniors. Care and Support to ensure seniors with needs are supported. Provide information and referral service and follow-up with referrals made. Participate in discussion with case management team to ensure clients are served holistically. Others Support in volunteer management processes e.g. keep track of volunteering activities and hours, maintain volunteer database. Administrative duties which include data collection and reporting for reporting and evaluation purposes. QUALIFICATIONS Minimum GCE 'O' levels, up to Diploma graduates. IT literate Good communication and social skills to work with d
