This is a non-bedside role for a Licensed Practical Nurse (LPN) to support patients enrolled in Medicare’s Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs. Your Contribution: Create, update, and review patient care plans based on the provider’s plan of care Conduct monthly telephonic check-ins with patients to assess health status and medication adherence Review RPM synced vital signs and escalate concerns to the provider when appropriate Coordinate follow-ups, scheduling, and documentation updates Ensure all documentation meets Medicare CCM and RPM compliance guidelines Accurately document all patient interactions within multiple EHR systems Your Qualification: Active LPN License (Multistate Nurse Compact License Required) Strong verbal and written communication skills Minimum 1 year in related Chronic Care Management services Experience working with EHR systems Ability to work independently with minimal supervision Reliable high-speed internet and a quiet, HIPAA-compliant workspace What you can expect from us: Purpose : Become a part of a significant initiative.
