Posting Date 06/03/20266902 Se Lake RdSte 100, Milwaukie, Oregon, 97206-4546, United States of America We are looking for a highly organized and detail-oriented Integrated Kidney Care Advocate I (IKCA I) to join our amazing team. The IKCA I is responsible for identifying and validating hospital admissions for Transition of Care (TOC) support, as well as providing vital administrative and communication support to our care teams. In this role, you will be collecting, verifying, and entering information into IKC databases to help ensure our IKC Model of Care operates smoothly. Candidate must live in Pacific or Mountain Time Zone. Essential Duties & Responsibilities: Transition of Care Support: Confirming the status of existing admissions, identifying new admissions and discharges, conducting 2x weekly SNF check-in calls to confirm admission status, and requesting admission and discharge summaries upon confirmation. Care Team Admin Support: Addressing ad hoc TOCs and SNF admits, requesting, obtaining, and uploading medical records (including labs), completing manual mailing requests, and conducting provider verifications. Administrative Communication Support: Providing administrative support to the care team, including monitoring incoming faxes, processing outbound faxes, and maintaining the fax log. Other Duties as Assigned Required Qualifications & Proficiencies: High School Diploma required 2 years of clinical administrative experience (Preferred). Intermediate proficiency in MS Word and Excel (Preferred). Excellent written and verbal communication skills to successfully perform essential functions.
