Job Description
Plan, promote, and implement wellness programs and activities at the community centres. Conduct screenings, physical assessments, and track patient progress with follow-ups. Monitor patient participation and compliance with individualized care plans. Maintain and disinfect program equipment regularly. Collaborate with clinical teams to support patient engagement and wellness outcomes. Requirements: Diploma in Health Sciences (Health Management), Sports Science, or equivalent. At least 2 years of experience in healthcare, community care, or wellness program facilitation. Passionate about community service with a proactive and organized approach. Strong interpersonal and communication skills.
