Job Description
Associate Psychologist /counsellor for Nursing Home provides basic psychologist /mental health assessment and support vital mental health and emotional care to elderly residents, often those with dementia or chronic conditions, design and conduct group therapies. Job Responsibilities · Coordinate and conduct preliminary and routine mental health assessments for residents. · Maintain documentation and records for services provided to cases. · Evaluate residents' mental health, abilities, and interests using a person-centered approach to support psychological and emotional well-being through: a) Individual Support: Provide one-on-one counselling and behaviour management. b) Group-Based Interventions: Design and facilitate therapy sessions and therapeutic activities. c) Collaborative Care: Work with stakeholders (CTS, Pastoral Care, MSW, etc.) for holistic support. · Provide case management by supporting and coordinating care and resources to address residents' needs and achieve desired outcomes. · Crisis screening and management. · Participate in Multi-Disciplinary Team (MDT) meetings and case discussions, contributing psychological insights/interventions. · Attend and present findings (if required) in Staff and Management meetings, etc. · Engage in continuous professional development and training. · Assist in other duties and functions as assigned by the Reporting Officer.
