Position Overview As a Clinical Nurse Coach, you'll be responsible for providing remote health coaching to members who are either risk for chronic kidney disease or already have chronic kidney disease. You'll work in partnership with our members to increase their understanding and management of conditions and behaviors that impact kidney health, helping them take charge of their health and avoid dialysis. This role in particular will work three late shifts per week, especially to accommodate our members who live on the west coast. You'll need to be fluent in both English and Spanish languages to support our members. How You’ll Contribute Establishing and maintaining contact with assigned and active members via video, phone, text, and email in both English and Spanish. Obtain and utilize clinical information to develop an individualized member care plan and clinician-centered care plan per NCQA guidelines. Assess members health status and care coordination needs. Collaborate with members’ health care providers as needed. Utilizing motivational interviewing techniques, individualized care plans, and the Triple Aim framework to empower active members to reach their health goals.
